The Brant Community Health Hub offers an innovative model of
care that will house a wide range of community health care
and services under one roof.

With your help and generous donations, residents of all ages will
benefit from having integrated programs close to home.
Improving access to health care and community services in the
County of Brant has been identified by the County of Brant
Council and the Healthcare Community as a vitally important and
growing need. Together, we will build a healthy community.

“The Brant Community Health Hub will provide our community
with a stable framework for a healthy community and reduce
the burden on an already overextended hospital system.
Residents from newborns to seniors will be able to benefit from
integrated programs, focused on their needs, close to home.”

~ Mayor David Bailey

A Place for All..

The County of Brant is proud to launch its Building a
Healthy Community Campaign for the Brant Community
Health Hub.
This leading-edge facility is like no other in Ontario.
It’s much more than just a medical clinic – it will offer
all-encompassing services that will meet your physical,
mental and health care needs. It’s a place for family
doctors, medical professionals such as physiotherapists,
chiropractors and psychologists, diagnostic services
including blood work and x-rays, mental health services,
onsite pharmacy, cardiology, optometry, audiology and
government services such as family and children services.
The Hub will also be a place for people to connect with
others in the common indoor areas such as the community
kitchen and outdoor spaces. It’s a one-stop shop for
everyone in the County. It will be a facility with expanded
capacity to serve you, your family, friends and neighbours,
now and in the future.
The Hub will engage the broader community by
strengthening capacity in primary care and expanding the
breadth of services in the County of Brant.

The Brant Community Health Hub, located at 25 Curtis Ave. N., Paris, will be approximately 41,365 square feet. It will offer a range of community health care and services under one roof including family physicians, health care practitioners, community support agencies, health supplies, and other health care and social services. Tenant stakeholders will include PrimaCare Community Family Health Team, Cobblestone Medicine and Rehab Centre, Brantford Radiology, Life Labs, Sound Choice Hearing Health Services, Northville Pharmacy, Grand River Community Health Centre, Alzheimer Society, Nova Vita, Participation Support Services, Brant Family and Children’s Services, Paris Heart Clinic and a number of other related health and social service agencies.
